Description of Services: Support for young adults who are, or have been, involved in the Child Welfare system and are transitioning to independent living * connects individuals with resources available in their community

Assists young adults in identifying, developing and implementing a plan based on personal goals including strategies to address:
  • Educational resources and post-secondary opportunities
  • Employment services and training
  • Life skills training
  • Health, mental health and other social services supports
  • Volunteer and recreational activities
  • Housing
  • Safety planning
  • Legal services
