Description of Services: Supports for people 18 years and older at risk for diabetes, living with prediabetes, type 1 diabetes, type 2 diabetes, or who are at risk for diabetes * one-on-one or group sessions * education and management support on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff
