Alternative to institutionalization for older adults and adults with disabilities who wish to continue living in the community but who require support services beyond that which can be provided through Ontario Health atHome and other community-based services

Services can also be provided to clients in their own homes who live East of Adelaide and North of Oxford

Eligibility - Population(s) Served: Older adults and adults with disabilities, who need physical, social, or emotional supports * VON care coordinators assess the needs of the applicants
