Program providing trained peer support and drop-in centre Peer Support Program - provides trained peer supporters with lived experience and now in recovery * peer supporters assist through:
Transitional Discharge Model (TDM) - Peer support service for individuals experiencing major life transitions such as moving from hospital to community while recovering from a mental health challenge * peer support mentoring may include assisting with problem-solving, learning a new skill or sharing tools and resources Outpatient Peer Support Group - For those recently discharged from hospital * program includes goal setting, discussion on challenges and accomplishments since discharge |
