Description of Services: Assists and coordinates transportation to appointments and other health care services outside of the community

Eligible services include:
  • to and from an approved medical facility
  • to and from a recognized health centre or specialized clinic when referred to by a medical doctor
  • to and from dental appointments
  • to and from optometrist appointment for testing and initial fitting
  • to attend long-term treatment (kidney dialysis, cancer care treatments)
  • prenatal/postnatal care appointments
  • physiotherapy if covered through OHIP/NIHB
