Description of Services: Companionship, respite care, and emotional support to people and their family/caregivers coping with serious illness or declining health* bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes

In-Home - Screened, trained and experienced Volunteers provide in-home visiting and Nav-CARE. Services provided in confidence and may include companionship, respite care, practical and emotional supports * service can be weekly or more frequent depending on need

Bereavement Services - Bereavement Support Groups held in various communities twice annually, and one-to-one grief support.

Eligibility - Population(s) Served: Clients diagnosed with a progressive life-limiting illness or grieving the death of someone close to them
Application: Call or email, self-referrals accepted, from family, health care providers
